Electronic Disclosure and Consent

Please read this Electronic Disclosure and Consent (the “Consent”) carefully and retain a copy for your reference. Citizens ("Citizens Bank") is a brand name of Citizens Bank, N.A. In this agreement, "we", "us" and "our" mean Citizens and "you" and "your" means each account holder, product owner and/or service user identified on an account, product or service.

Consent. You Consent to receive all Tax Documents for every account, product or service you have with us electronically until (a) you withdraw consent by changing your election, (b) the relationship between you and us is terminated or (c) we notify you that we will no longer furnish your Tax Documents electronically. Your Consent may not be partially granted or revoked. For a list of all Tax Documents provided electronically please sign into your online banking at Citizensbankonline.com and visit Document Center. Tax Documents will be delivered to you electronically through our website. Even with your election, you may obtain paper copies without revoking consent by contacting 1-800-656-6561. Fees may apply to receive a paper copy. Please see your deposit account agreement and accompanying fees and features guide for applicable fees, or call us at 1-800-656-6561 (personal account customers) or 1-877-229-6428 (business customers) to determine applicable fees.

Withdrawal of Consent. You may withdraw your Consent to receive Tax Documents electronically at any time. To make a change and receive paper copies, visit Citizensbankonline.com, access account details, click on the Tax Forms Edit link, change the Go Paperless selection to Turn On Paper and click the Save Changes button, or write to us at Deposit Tax Compliance: Citizens Bank, ROP230, 1 Citizens Drive, Riverside, RI 02915. Changes will not take effect for documents already issued nor affect the legal validity and binding effect of any Tax Documents you received, electronic or otherwise, before the effective date of the withdrawal of your consent. You will not be charged a fee if you withdraw your consent. We will send you confirmation of the withdrawal, in the manner the request is received. Additionally, the Bank reserves the right to revoke your consent to receive Tax Documents electronically at any time. In the event of termination by us, we will notify you that your electronic consent is revoked and you will receive paper copies.

Updating Your Contact Information. You must have an active e-mail address in order to use the Service. You must promptly notify us of any change in your e-mail address or other contact information. You may change/update your personal information by calling 1 800-656-6561. Your email address may also be updated by accessing “My Profile in OLB”. If you fail to update or change an incorrect or invalid e-mail address or other contact information, you understand and agree that all Tax Documents shall nevertheless be deemed to have been provided to you if they were made available to you in electronic form or delivered through other electronic means.

Hardware and Software Requirements. To receive Tax Documents in electronic form, you must have an electronic device equipped with a connection to the Internet; (ii) an Internet web browser which is capable of supporting HTML and 128-bit SSL encryption and be JavaScriptTM enabled; (iii) Adobe Reader version 7.0 or higher (If you do not have Adobe Reader, you may download a free copy at https://get.adobe.com/reader/); and (iv) an HTML enabled email account. To retain a copy of these electronic communications, you will need to be able to either download electronic copies to your computer or an attached storage device (such as a disk or USB storage device), or print copies on a printer attached to your computer.

You will automatically default to paper copies unless you consent to go paperless.
